click the Forgot your User ID or Password link

enter the following details:

employee number
company number – these numbers can be found as the 2 digits Hospital Code used during your account registration. Most users the numbers are 01 but you’ll want to check with your hospital’s payroll or human resources department for verification.
last 4 of your social security number
email address
click the RESET button
