Alconchoice – Rebate Form Submission Online

Customers that purchased contact lenses made by Alcon Laboratories can receive a rebate on their purchase by visiting the Alcon Consumer Rebates Center online.  Rebate claims can be initiated from the website 24 hours a day or consumers can opt to download a form, complete it, and return by mail.

Like mentioned there are 2 ways to submit your rebate form which is online or by mail.  Here is a simple guide with info on how to do both:

Online Form Submission

• Visit www.alconchoice.com

Before you visit the AlconChoice website you should have handy a few items to help with completing the online form:

• 1 original UPC code for each brand of lens purchased
• A copy of your eye exam receipt and or contact lens fitting receipt
• A copy of the original sales receipt with the eligible contact lens products circled

It only takes a few minutes to complete the online form so at the website let’s get started:  

• Enter your email address
• Click the blue Continue with Email button
• Enter your first name
• Enter your last name
• Enter your password
• Click the blue Register
• An email verification will be sent to your email inbox
• Click the verification link in your inbox
• Enter your login credentials
• Properly setup your rebate account
• From inside your online account you can start your claim and return to check on your claim status

At alconchoice.com you will need to upload the above mentioned items with your claim:

• UPC, eye exam receipt/contact lens fitting receipt sales receipt
• Unique rebate code
• Required contact info

After you finish submitting the rebate online you will receive an email from Alcon with instructions on obtaining your rebate reward.  You will have the option to select how your rebate will be delivered to you.

Mail in Your Claim

The online option is much faster and your payment is processed quicker but for individuals who prefer the mail in route you can follow these instructions:  

Download an AlconChoice Rebate from to print at www.alconchoice.com.

Include the same items as the online process:

• 1 original UPC code for each brand of lens purchased
• A copy of your eye exam receipt and or contact lens fitting receipt
• A copy of the original sales receipt with the eligible contact lens products circled

***Recommended to take photocopies of your entire submission for your records
