Employees with short term disability and Family & Medical Leave health coverage via Lincoln Financial Group can utilize the online resources to file a claim or leave. The MyLincolnPortal site is accessible 24 hours a day 7 days a week for employees to utilize.
It is a self service benefits site that allows users to submit a claim or leave of absence and track their status of their claim.
How to Report a Claim/Leave
- Visit www.mylincolnportal.com
- Enter your Username and Password
- Click the blue Log In button

How to Register Online
If this is your first time using MyLincolnPortal you will need to register – follow these steps:
- Click the New user? Register for an account link
- Enter your Company Code
- Click the blue Validate button

Click the Forgot username or password link
Enter your username
Click the blue Submit button

Your information will be pre-filled from your user profile
Enter Details About Absence

Claim or leave info with reason and dates of medical or other authorized events
Days you will be absent
Circumstances for your absence
Last day of work expected
Physician or hospital contact info
Physician’s name and phone number
Review
Review all of the information you’re submitting and make any changes necessary
Submit
After submitting you will receive your claim number and claim file contact information
When reporting a claim or leave you should have the following documentation available:
- Physician or medical care provider’s name, address, fax, and telephone numbers
- Manager’s name, telephone number, and email address
- Reason you’re out of work
- Your last day worked, first day absent from work, and anticipated return to work date
