With the YourHealthFile patient portal users will be able to review all of their health information from one location.  They can receive a summary of all of their visits, medications prescribed, request and schedule appointments, and more.  

Your health information is readily accessible from your home computer, mobile devices, or smartphone.  The first steps in getting started with your account is going through the activation process:

How to Activate YourHealthFile Account

• Visit www.yourhealthfile.com activate my account
• Click the orange Activate Your Account button

Enter the following details:

• Username
• Password
• Date of birth
• Click the blue Submit button

• Communicate with your primary care
• Receive and review detailed summaries of your health record
• Keep track of your allergies
• Medication review
• Receive lab test results
• Request and schedule virtual visits
• Upload files and medical records to your chart

How to Register YourHealthFile

• The first step to registering for YourHealthfile is to receive an invitation from your healthcare provider.
• Wait to receive a printed invitation from your healthcare provider
• Visit yourhealthfile.com activate my account from your home computer, mobile device, or smartphone
• Click the “Activate your account here” button above the User Login form
• An Account Activation screen will display
• In the “Username” field enter your temporary username and password
• Enter your date of birth
• Click the “Activate Account” button
• After you receive confirmation, the Update Account screen will display and you will be asked to enter your desired account details
• Complete the Update Account form
• Click the “Update Account” button

Logging In

When you login for the first time the system will show a check box asking you to confirm that you’ve read and understand this document and any information I supplied is true and accurate.
